När du bokar behandling

Bokningsformuläret samlar in de uppgifter du själv lämnar, till exempel för- och efternamn, telefonnummer, e-postadress och information i fritext om vad du vill ha hjälp med. Uppgifterna används för att hantera och följa upp din bokningsförfrågan, bedöma om och hur behandling kan erbjudas samt kontakta dig om nästa steg och möjliga tider.

Lämna bara uppgifter som är relevanta för bokningsförfrågan. Använd inte vanlig e-post för känsliga personuppgifter eller detaljer om ditt hälsotillstånd.

When you book therapy

The booking form collects the information you provide, such as your first and last name, telephone number, email address and information in the free-text field about what you would like help with. The information is used to manage and follow up your booking request, assess whether and how therapy can be offered, and contact you about the next step and possible appointment times.

Only provide information that is relevant to your booking request. Do not use regular email for sensitive personal data or details about your health.

CTNotes

Formuläret och journalsystemet tillhandahålls genom CTNotes, en tjänst från AB Tjugofyra se. CTNotes behandlar personuppgifter för verksamhetens räkning som personuppgiftsbiträde. När formuläret laddas ansluter din webbläsare till CTNotes, vilket innebär att tekniska anslutningsuppgifter kan behandlas för att leverera och skydda tjänsten.

CTNotes

The form and record system are provided through CTNotes, a service from AB Tjugofyra se. CTNotes processes personal data on behalf of the practice as a data processor. When the form loads, your browser connects to CTNotes, which means that technical connection data may be processed to deliver and protect the service.

CTNotes states that information is stored in Sweden and that communications and databases are encrypted. Read more on the CTNotes website.

Rättslig grund

Uppgifter i en första förfrågan behandlas för att på din begäran kunna hantera en möjlig vård- och behandlingskontakt. När uppgifter behöver behandlas för vård, behandling eller journalföring sker det för att tillhandahålla hälso- och sjukvård och uppfylla rättsliga skyldigheter enligt bland annat patientdatalagen. Hälsouppgifter behandlas endast när det är nödvändigt för hälso- och sjukvårdsändamål och under den tystnadsplikt som gäller för legitimerad hälso- och sjukvårdspersonal.

Legal basis

Information in an initial enquiry is processed in order to take steps at your request towards a possible healthcare and therapy relationship. When information needs to be processed for healthcare, therapy or record-keeping, it is processed in order to provide healthcare and comply with legal obligations, including those under the Swedish Patient Data Act. Health data is processed only when necessary for healthcare purposes and under the duty of confidentiality that applies to licensed healthcare professionals.

Vilka som kan ta del av uppgifterna

Endast personer som behöver uppgifterna för sitt arbete i verksamheten får ta del av dem. CTNotes behandlar uppgifter för verksamhetens räkning. Uppgifter kan också lämnas ut om det krävs enligt lag eller ett myndighetsbeslut. Uppgifterna säljs inte och används inte för marknadsföring.

Who may access the information

Only people who need the information for their work in the practice may access it. CTNotes processes information on behalf of the practice. Information may also be disclosed if required by law or an official decision. The information is not sold or used for marketing.

Hur länge uppgifterna sparas

En bokningsförfrågan som inte leder till vård sparas bara så länge den behövs för att hantera och följa upp förfrågan och raderas därefter enligt verksamhetens gallringsrutiner. Om uppgifter blir en del av en patientjournal gäller reglerna för journalhandlingar. En privat vårdgivare ska normalt spara en journalhandling i minst tio år efter att den sista uppgiften fördes in.

How long information is retained

A booking request that does not result in healthcare is retained only for as long as needed to manage and follow up the request and is then deleted in accordance with the practice's retention procedures. If information becomes part of a patient record, the rules for medical records apply. A private healthcare provider in Sweden must normally retain a medical record for at least ten years after the last entry was made.

Dina rättigheter

Du kan bland annat begära information om vilka personuppgifter som behandlas om dig och begära att felaktiga uppgifter rättas. Beroende på situationen kan du också ha rätt att begära begränsning eller radering eller att invända mot behandlingen. Rätten till radering är begränsad när uppgifter måste bevaras enligt reglerna om patientjournaler.

Your rights

You may request information about the personal data processed about you and ask for inaccurate information to be corrected. Depending on the circumstances, you may also have the right to request restriction or erasure, or to object to the processing. The right to erasure is limited where information must be retained under the rules governing patient records.
